Insurance Referrals
If you need a referral, please call your Primary Care Physician and have them fax one to us at (248) 465-2850 prior to your visit.
Cancellations
Please call our office (248-465-1200) 24 hours in advance if you need to reschedule or cancel your appointment to avoid a $25 cancellation fee.
